PLEASE READ OUR TERMS AND CONDITIONS BELOW BEFORE REGISTERING FOR A COURSE:
Cancellation and Rescheduling: We understand that emergencies occur. In the case you are unable to attend the class, you may reschedule within 14 business days of the scheduled class. We ask for 24 hours' notice when possible. You may reschedule the class up to 2 times and it must be completed within 30 days of initial registration.
No call, no show: No store credit or refund will be honored if we do not hear from you within 14 business days of the class schedule.
Refunds: No refunds allowed. Once paid for a class or product, we do not offer any refunds. You may request a store credit and is approved at the manager's courtesy. If a store credit is offered (in rare cases), the credit would expire 30 days after the initial registration date. No Refunds, exchange, or store credit will be applied for any online classes and/or electronic book purchases.
Success Policy: We offer a 100% success guarantee to all of our classes. In case you fail a class, you will be offered to take a make-up test or retake the class without any additional cost.
Same-Day Certification: All of our courses come with same-day certification upon successful completion of the course. All American Heart Association courses are issued a two years certification.
Arriving to class: We want you to succeed. Please plan on arriving 15 minutes before class. Per American Heart Association and our policy, you will not be able to attend the course after 10 minutes of class start time. Please call us when an emergency arises and we would be happy to reschedule you.
Books: Some courses require you to have the appropriate class books. We do sell all class books at our facility, but you may bring your book if you have it.
Communications: By providing your phone number and email, you consent to receive text messages and emails from Health Street. These may include class confirmations, updates, reminders, and notifications when your certification is nearing expiration. At any time, you can respond with the word STOP to stop all future communications.
Minor Policy: We do allow children under 18 to take the basic courses. The American Heart Association research shows that children as young as 9 years old have been able to properly perform compressions. The minor would need to be able to perform the skills on the mannequin - including compressions, AED, and choking training. It is at the sole discretion of the parent/guardian to decide whether you believe your child would benefit from the training. If you do decide to register your child, you will need to present at check-in. A minor form must be filled out for the child as well, this can be done digitally or at Health Street LLC. You can find a digital copy of the minor form here: MINOR WAIVER FORM. You can email this form prior to the course to info@cprologist.com, but please note that the parent/guardian must be present at check-in, even if the minor form has already been submitted. Please also make a note in the comments section during registration that the participant is a minor so that we can let the instructor know. Thank you!

Onsite Course Terms & Conditions
Onsite course is defined as when Health Street instructors come to your facility for the course.
Deposit:
A $200 deposit is required to secure your onsite course date. This deposit is nonrefundable and nontransferable but will be applied toward your total balance unless the class is rescheduled.
Travel Surcharge:
A $50+ travel fee applies for locations more than 25 miles from Health Street. Exact travel costs will be confirmed at the time of booking.
Repeat Customer Discount:
We value our returning clients! Receive 5% off your next onsite session if booked within 12 months of your previous class.
Payment:
Payment is due on the day of the course.
